General Overview of Transition:
Parents and Caregivers: Transition is a time in your student’s life ranging from 14-22 years of age. This is the time when your student and family will develop a plan for their future. A Transition can be unsettling. You may be thinking, “I finally have the IEP process down. I finally understand what questions to ask my student’s case manager.” Then you see adulthood racing towards your child with all the uncertainty it may bring.
Don’t worry! Take a breath…there is an abundance of resources for you students and family to make this transition smooth and find the best possible outcome for all.
