Assessment Procedures

1. Choose a Qualification: Select one of the Accredited Qualifications offered at TESDA PTC Rosario 

2. Download and Fill Out Forms: Obtain and completely fill out the Application Form and the Self-Assessment Guide. (downloadable forms are located below the page)

3. Submit Documents: Submit the completed forms and all other required documents to the TESDA Provincial Training Center Rosario office at Phase 1, Cavite Economic Zone, Rosario, Cavite.

• 2 pcs Passport size picture (white background, attire with collar and with nameplate, chemical print)

• Practicing Covid-19 Preventive Measure in a Workplace Certificate from E-tesda Online Program (www.e-tesda.gov.ph)

• Certificate of Employment if applicable

4. Confirm Schedule: A TESDA PTC Rosario Processing Officer will contact you by phone to confirm your assessment date and time.

5. Pay and Receive Admission Slip: After confirmation (Steps 1-4), you must pay the approved assessment fee to the cashier at TESDA PTC Rosario. You will then be issued an admission slip containing your official assessment schedule.
