適應症：

• 低血鎂 【1️⃣1支in N/S or D5W 100mL run 1 hr】【2️⃣2支  in N/S or D5W 250-500ml run】

  • Mild (>1.5 to 1.9 mg/dL) 1-2g over 1 to 2 hours 

  • Moderate (1 to 1.5 mg/dL) 2-4g over 2 to 12 hours 

  • Severe (<1 mg/dL) 4-8 g over 4 to 24 hours 

  • Symptomatic patients (eg, tetany, arrhythmias, seizures) (excluding torsades de pointes and eclampsia/preeclampsia):  

    • unstable 1-2g 快速 bolus over 2 to 15 minutes (150mg/min)

    • stable 1-2g iv bolus over 5-60 minutes, followed by an additional 4 to 8 g over 12 to 24 hours 

• Preeclampsia

  • IV

    • 4-6 g loading dose over 15-30 min at onset of labor or induction/cesarean delivery, followed by 1-2 g/hr continuous infusion for at least 24 hours after delivery; Max infusion rate: 3 g/hr.  

    • If seizure occurs while receiving Mg, an additional bolus of 2-4 g may be administered over ≥5 min with frequent monitoring for toxicity. 

    • Maximum dose of 40 g per 24 hours 

  • IM: (建議是用50%濃度的, 或無法IV時) 5 g in each buttock (共10g), followed by 5 g every 4 hours for at least 24 hours after delivery.  

• Polymorphic VT (with pulse) associated with QT rolongation (TdP): 1-2 g (diluted in 50-100 mL D5W) over 15 min. If no response or torsades de pointes recurs, may repeat dose up to a total of 4 g in 1 hour; may follow with a continuous IV infusion of 0.5-1 g/hr   

• VT/pusless VT due to TdP (二線用藥):  1支稀釋10 mL D5W bolus over ≥1 to 2 min; if ineffective, may repeat immediately with 2 additional 2 g bolus doses as needed; Max total dose: 6 g; use IO route if IV not available 

• Bronchospasm refractory to bronchodilation (For severe or life-threatening): 2 g as a single dose > 20 min.
