The Technology Assisted Children's Home Program (TACHP), in partnership with the National Nurse-Led Care Consortium, hosted a virtual training for families and nurses on May 23rd, 2023. Providing in-home health care is an intimate and dynamic partnership between healthcare staff and families. Implementing robust whole-child/family-centered communication and care allows a nurse to enter a patient’s home better prepared to care for and support each individual child in their environment. Utilizing Vision for Equality's “Beyond the 485” tool as a guideline, participants gained strategies for stronger, clearer communication between home health agencies, shift nurses, and their patient families.
