Technology Assisted Children's Home Program (TACHP)
TACHP is a free program developed to provide unique, home-based care coordination to parents of children with special healthcare needs. TACHP provides non-medical services to build capacity among families and caregivers to care for their technology assisted children in their home. Our team of community navigators draw on the shared expertise in the service areas provided within the program, including special education, parental advocacy, medical assistance, accessing in-home nursing services, etc.
HPC - Health Promotion Council possesses more than 40 years of experience in chronic disease prevention and management, including asthma, diabetes, cancer, hypertension, nicotine addiction, and obesity. We reach more than 20,000 people across Pennsylvania each year through direct service programs targeting the reduction of health disparities, particularly among low-income and racial/ethnic minority populations.
Our work focuses on improving health outcomes among vulnerable populations. As such, their core values include:

The National Nurse-Led Care Consortium is a nonprofit member-supported organization working to strengthen community health through quality, compassionate, and collaborative nurse-led care.
The NTTAP team at NNCC delivers training and technical assistance to public housing primary care health centers to enhance clinical practice and improve skills and capacity of health centers to meet health care needs of residents of public housing.
