Patient Satisfaction Survey

Every year as a part of being connected to the clinic, we survey the patients that come use clinic services. We ask them about their demographics, their opinion on the services they used and things the clinic could improve upon. We use data collected from our survey to share key results with the staff at the clinic. This allows the clinic to see what they can improve upon and what they are doing well. This year we modified the survey to include questions on COVID-19 and the clinic's safety measures.
