To fill it in online, you will need to download it to your device and then open it using Adobe Acrobat Reader, or which ever PDF reader you choose. The form has text boxes for you to fill in your information. 

Once it is filled in, save it to your device, then email it to our Parish Secretary, Mary Murillo, marymurillo@seascc-ont.org, at the Parish office.
