Sponsor Registration

Ready to invite your sponsor? Please have him/her fill out the attached form, no later than November 7, 2025. If you are having 2 sponsors , each person must fill out her/his own form. You can:

~ simply share this website with him/her, OR

~ click the button for the form yourself and share the URL for the form with him/her by copying and pasting it from the address bar.

~ open the form and fill it out while "on the phone" with your child's sponsor

If you would prefer a printed copy of the form, please contact the parish office. You will be responsible for providing the completed form to the office by Nov 7

Once a form is submitted, the child's first name will show on the sheet linked below. Click the button to see if your sponsor has successfully submitted the form.

Who has submitted Sponsor Forms?

Want to have a sponsor that cannot physically attend the Sacrament of Confirmation? You can use a PROXY. Your sponsor still needs to fill out the form though, so go ahead and send it :) Read the red text at the top of the previous sheet.
