Sponsor Registration

Ready to invite your sponsor? Please have him/her fill out the attached form, no later than November 7, 2025. If you are having 2 sponsors , each person must fill out her/his own form. You can:

~ simply share this website with him/her, OR

~ click the button for the form yourself and share the URL for the form with him/her by copying and pasting it from the address bar.

~ open the form and fill it out while "on the phone" with your child's sponsor

If you would prefer a printed copy of the form, please contact the parish office. You will be responsible for providing the completed form to the office by Nov 7.

Want to have a sponsor that cannot physically attend the Sacrament of Confirmation? You can use a PROXY. Your sponsor still needs to fill out the form though, so go ahead and send it :)

Not sure who to be a sponsor or don't really have anyone in mind? Feel free to contact any member of the Sacramental Prep team for ideas :)
