Ready to invite your sponsor? Please have him/her fill out the attached form, no later than November 16, 2022. If you are having 2 sponsors , each person must fill out her/his own form. You can:

If you would prefer a printed copy of the form, please contact the parish office. You will be responsible for providing the completed form to the office by Nov 18.
Want to have a sponsor that cannot physically attend the Sacrament of Confirmation? You can use a PROXY. Please contact the Preparation Team no later than Nov 18 to discuss this. Your sponsor still needs to fill out the form though, so go ahead and send it :)
