Upon identifying and prioritizing the top three health problems in Barangay Lindang, intervention plans are developed through the Community Health Assessment Plan (CHAP). This process involves carefully strategizing and designing targeted programs and activities aimed at addressing each health issue. By integrating community participation and evidence-based approaches, CHAP ensures that interventions are both relevant and sustainable, ultimately improving the overall health and well-being of Barangay Lindang residents.
TOP 1: High Prevalence of Hypertension
Cues:
Hypertension ranks as the 1st cause of morbidity in the barangay as of 2023.
Three reported mortalities in the year 2024 due to acute myocardial infarction (the second leading cause of mortality in 2023) caused by chronic hypertension.
As of 2024, there are a total of 63 diagnosed hypertensive residents in the barangay.
As of 2024, only 5 residents with hypertension undergo monthly blood pressure monitoring.
The barangay health station is inadequately equipped for blood pressure monitoring as its stethoscope and sphygmomanometer are nonfunctional.
As of 2024, risk assessment of residents 20 years old and above identified multiple factors contributing to an increased risk of hypertension.
Out of 235 individuals, 32 (13.6%) have a family history of hypertension.
Out of 235 individuals, 80 (34.04%) are overweight, 15 (6.38%) are obese class 1, and 1 is classified as obese class 2.
Out of 232 individuals, 105 (44.7%) have a waist circumference above normal standard.
Out of 232 individuals, 57 (24.4%) are smokers.
Out of 232 individuals, 103 (43.4%) are alcoholic drinkers, 16 (6.8%) of which are regular drinkers.
Out of 232 individuals, 73 (31%) consume more than the WHO recommended dose of sodium consumption per day.
Among the 63 diagnosed hypertensive residents, 18 (28.5%) do not take maintenance medications, and 11 (24.4%) among the 45 hypertensive residents taking maintenance medication have poor compliance with a Morisky Medication Adherence scale of 0 and 1.
None of the residents utilize the proper herbal plants as management and adjuncts for treatment of hypertension.
Out of 45 hypertensive individuals on maintenance medication, 26 (57.77%) had uncontrolled blood pressure readings on two separate occasions despite taking antihypertensive medications.
General Objective:
To reduce the prevalence of hypertension from 15% to 12% among individuals 20 years old and above in Barangay Lindang by the year 2027.
Specific Objective:
Enhance the availability and accessibility of resources for hypertension management.
Strategies:
Establish a Barangay Health Board for the procurement of the necessary equipment and supplies at the Barangay Health Station.
Increase the knowledge of hypertensive individuals regarding pharmacological and nonpharmacological management of hypertension and the importance of adherence to maintenance medications.
Strategies:
Educate the residents with known hypertension on the management of hypertension and importance of adherence to maintenance medications.
Alagang Puso seminar on the Importance of Proper Monitoring and Adherence to Hypertensive Maintenance Medications.

TOP 2: Prevalence of Malnutrition among children 0-59 months old
Cues:
The Barangay Nutrition Scholar recorded under-five Malnourished in the community. Specifically, these are the numbers:
12.77% (6/47) of children aged 0-59 months were reported to be stunted and 4.26% (2/47) were severely stunted.
2.13% (1/47) of children had Moderate Acute Malnutrition (MAM).
6.38% (3/47) of children were found to have borderline MAM.
8.51% (4/47) of children were underweight and 2.13% (1/47) were severely underweight.

The 15 - 19 year old population makes up the second most in the total population of Barangay Lindang (10%) and is most at risk for recurrence of teenage pregnancy.

Only the Seventh-Day Adventist faith supports the use of hormonal contraceptives.
Service Delivery
Adolescents as young as 15 can access contraceptives through the Barangay Health Worker (BHW) with proper guidance.
