Mandatory Forms

You must sign the Participant Liability Form before coming to any practice.

If you do not sign this form, you will not be allowed at any practices until you have agreed to its terms. You will need to show the emailed receipt* of the Participant Liability Waiver at the beginning of every practice, so please do not delete it.

You will only need to sign the Waiver once during the academic year.

Club Roster Form

All members are required to fill out this form after attending a COFA practice. This form is managed by the Campus Recreation Center to maintain an accurate roster of all Sports Clubs. COFA does not have access to the responses on this form.

Club Roster Form

If you have your own equipment and plan on bringing it to campus you must fill out the Personal Equipment Form

Please fill this form out if you have your own equipment (i.e. risers, limbs, arrows, essential accessories, bags, etc). This policy does not apply to people who only own finger tabs and armguards. If you would like access to this form please contact us by email or talk to us at practice.

If you buy new equipment that will need to be stored in the closet, please edit your initial submission to include the new item(s). If you did not receive an email with your submission receipt, please email us at archery_club@stonybrook.edu so that we can update it from our end.
