This link allows people to become more informed about their community and determinate that effect it. Ontario Public Health has developed a forum that outlines at risk communities and further educates community member. The common issue within community health is the lack of awareness and involvement from community members themselves to understand how social determinates of health effect individuals, communities and populations.
