To ensure the proper and secure release of Social Pension benefits, the following requirements must be submitted when authorizing a representative to claim the payout on behalf of the beneficiary:
Annex 12 - Authorization (DSWD Social Pension Form)
Affixed signature or thumbmark of both the beneficiary and the authorized representative, and witnessed by anyone from the following: BSCA President, Punong Barangay, Barangay Kagawad, OSCA, or C/LSWDO; (Must be completely filled out and NOT altered)
Photocopy of Senior Citizen (SC) ID
With three (3) specimen signatures, identical to the signature on the ID
Photocopy of a valid Government-issued ID of the Claimant
With three (3) specimen signatures, identical to the signature on the ID
Written authorization letter from the social pensioner indicating the name of the authorized representative and the reason(s) for inability to personally claim their stipend with duly affixed signature
Photo of a handwritten authorization letter
Must include the date and signature or thumbmark of the beneficiary
Photo of the beneficiary holding the handwritten authorization letter
✔ Letter must be clear and readable
✔ Signature must match the beneficiary’s ID
****These requirements are strictly implemented to prevent fraud and ensure that Social Pension benefits are released only to duly authorized individuals.****
The nearest surviving relative (next of kin) or a duly authorized representative (listed in 3 authorized representatives) may claim the Social Pension stipend of a deceased beneficiary subject to validation by the DSWD Field Office (FO).
Duly accomplished Warranty and Release from Liability Form (Annex 7)
Death Certificate (Original or Certified True Copy) issued by PSA or Local Civil Registrar
If unavailable, a Barangay Certificate issued by the Punong Barangay and attested by the BSCA President, OSCA, or C/MSWDO may be accepted, subject to validation.
For Indigenous Peoples (IP), Muslim, or Other Religious Sects
Certification from the Punong Barangay, Tribal Chieftain, and/or Imam attesting to the death of the beneficiary.
Photocopy of valid ID of the deceased beneficiary
Photocopy of valid ID nearest kin or authorized representative
With three (3) specimen signatures, identical to the signature on the ID
If the beneficiary passed away within the payment period (quarterly), the full stipend for that period shall be released:
₱3,000.00 – Quarterly
If the death certificate is not yet available during payout, a Barangay Certificate may be temporarily accepted.
The deceased beneficiary shall be delisted from the payroll starting the succeeding payment period.
