Reporting Athletic Injuries

It is important that all athletic injuries are reported immediately to the Coach or Athletic Trainer.


Injury Reporting Procedure:

1.Injury is reported to or witnessed by coach/athletic trainer.

2.Injury is assessed by athletic trainer and immediate care action is provided.

3.Injury report is submitted by coach or athletic trainer and kept on file in Nurse’s Office and Athletic Director’s Office.

4.If additional care outside of the school was utilized for injury, Region 8 School’s Athletic Insurance may be provided to cover those additional costs. The parent must contact the school to request a claim form. Once received and completed by it must be sent directly by the parent to the insurance company. The school will send injury documentation to the insurance company.
