Healthcare Provider (HCP) Shadowing: Observing clinical interactions to understand the application of patient-centered care principles and the challenges of navigating health within complex social realities.
Patient Follow-Up: Conducting longitudinal engagement to observe the journey of care over time and the profound impact of social determinants on health outcomes.
Community-Based Organization (CBO) Visits: Analysing the role of non-clinical, community-driven assets and partnerships in providing essential support and addressing the root causes of health disparities.
