S&I POLICY:
An individual must give voluntary, written consent before using his or her name, image, photograph, video, audio, interview, story, personal history, and any other recordings in any social media post or other materials. If the individual is a seminary student or minor child, consent must be obtained from the individual’s parent or legal guardian. Using a person’s name, likeness, or image without consent is contrary to Church policy and such use may also subject the Church or the user to legal liability.
To receive permission to use a student's image, provide the parent or guardian with a description of how their child’s image or information will be used. Instruct the parent or guardian to reply with the following information. Include a copy of the photo or recording as an attachment to the email.
I [parent / guardian] authorize (seminary program name) of The Church of Jesus Christ of Latter-day Saints to use [photo / video / audio / story / other recording] of [child's name] on [local social media or other materials].
