PMEA has transitioned to using an online form for student information, including medical information and policy agreements. Please note that the School Acknowledgment From and the Medication Administration Record (if needed) will still be paper forms.
This form must be completed for each student upon acceptance into the PMEA District 3 Jazz by a parent or legal guardian if the student is under 18 years of age.

Due to the small amount of rehearsal time, it is imperative that there be 100% attendance at ALL rehearsals and at the concert. Students are expected to have all music prepared for the beginning of the first rehearsal. Please take a moment to read through the festival policies and links as part of this form.
For an event of this type, it is necessary for PMEA to have emergency contact numbers and medical information for each participant. We sincerely hope that this information will never be used; however, in case of an emergency, it will be readily available. This information will be kept confidential.
