You can stay anonymous and only answer what you’re comfortable with. We’ve designed this form to help detect patterns across:
Biolife, CSL Plasma, Octapharma, Grifols, and other centers
False positives (including Hepatitis B, syphilis, or HIV)
Issues after recent vaccinations (including yellow fever, COVID, or Hep B)
Outdated lab procedures or incorrect deferral codes
Lack of grievance or appeal policies
