Conferences: Thursday, 10/23 - 4:00 - 8:00 PM; Tuesday, 10/28 - 4:00 - 8:00 PM. Teachers available starting at 4:30 PM each evening.
Athletic participation at PHMS is available to any 7th or 8th grade student. Any student participating in athletics must have a completed athletic participation packet on record in the school office before they will be permitted to practice or compete in any middle school athletics. The packet may be picked up in the school office or completed online. In addition, any 7th or 8th grade student who wishes to participate in middle school athletics must also have an athletic physical on record at PHMS. These forms can be downloaded from the district website or be picked up in the office. Middle school students are only required to complete one physical for their entire middle school career. However, any 7th or 8th grade student who is approved for petitioning up for a high school sport must have an athletic physical on record at the high school. Any student participating in high school athletics must have a new physical on an annual basis.

Students will need to be registered on Bound before practices start. All current game and practice schedules can be found on Bound. This is the only website that is currently being used by Patrick Henry Athletics.
Directions on how to sign up for athletics can be found by clicking here. Using those directions will help you get to the Bound website, create an account, and walk you through how to upload the proper documents.
Forms Needed to Participate in Patrick Henry Athletics
Complete the Activity Participation Packet. This needs to be completed prior to each school year.
Please login to your Parent Infinite Campus account and complete the “Annual Student Information Update.” After signing into Infinite Campus, click on “More” in the left hand margin to get to the Annual Student Information Update.
When completing the Annual Information Update, you must complete ALL children linked to your Parent ID.
Be aware that a green check mark by your student’s name does NOT mean that the information has been submitted. You must click “Save and Continue” all the way to the end of the program and then click “Submit”.
Complete the Preparticipation Physical Evaluation Form.
Your student will need a Preparticipation Physical Evaluation dated after April 1st of Students 5th Grade year and prior to participation.
This paper copy needs to be turned into the school office and it is valid through the end of students 8th grade school year.
Note: Your student will not be able to practice without these two required forms being completed
