Welcome to Patrick Henry Middle School! Enrolling your child(ren) into a new middle school can be a stressful with many hoops to jump through. We are here to help provide as smooth of a transition as possible. Below are the steps to complete for most families as well as things to consider before your child(ren)'s first day. As you go through the enrollment process, our registrar will communicate with you about enrollment confirmation and a start date. (Note: PHMS is currently closed to open enrollment consideration. Future consideration may be determined by completing the district open enrollment application.)
To begin, complete these easy steps on a laptop, desktop computer, OR at a kiosk at your child(ren)’s school.
When a student is enrolled in the Sioux Falls School District for the first time, families complete a Home Language Survey as part of the registration process. This form asks questions about the languages your child speaks and hears at home.
If the survey indicates that a language other than English is spoken, the student is referred for language screening.
Parents/guardians are notified in writing of eligibility within 30 days.
If eligible, please refer to the district English Learners Program page for additional information.
Submit Required Documents to PHMS
Birth Certificate: A certified copy of the birth certificate must be presented to the school at the time of enrollment or within 30 days. An affidavit issued by the State Department of Health may be presented in lieu of the birth certificate.
Immunization: Proof of immunization must be presented prior to admission to school.
Verification of address: One (1) document in the name of the parent/guardian clearly indicating the address is required to verify an address. Documents provided must be current and dated the month of enrollment date or previous month. Verification examples include: Lease Agreement, Utility Bill, Mortgage Document, or Homeowner’s/Rental Insurance. See District Regulation JCA-R for complete listing of allowed documentation.
Login to the Infinite Campus Parent Portal
Select “Annual Student Information Update” from the Shortcuts Section
Submit Address Verification to PHMS.

Next, determine whether your child(ren) needs any of the special services listed below. If a service is needed, follow the instructions on the given webpage. For assistance with navigation, contact our registrar.
If necessary, check the suggested School Supply List to prepare your child(ren) for their first day at PHMS. (If you need assistance with school supplies, please let our registrar know of your particular need and we will do our best to accommodate the need before your child(ren)'s first day.)
Finally, wait patiently for your documents to be received and processed. This can take 1-5 (or more) business days to complete, especially if you are transferring into the Sioux Falls School District. Our registrar will reach out to you once everything is in order to confirm your child(ren)'s enrollment to PHMS and discuss a start date that works best for your child(ren) and the natural pacing of the school year.
