MODIFIED SCHEDULE / EARLY RELEASE REQUEST FORM FOR SENIORS
To request late arrival or early dismissal, this form must be PRINTED, completed, and signed by a parent.
Students at Page High School are required to take 7 courses each year. The only exception is 12th grade students who are on track for graduation. Students requiring 6 or more credits to meet graduation requirements (22 credits) will NOT be approved. Students must be considered a "full time student," in order to remain eligible for athletics and driver eligibility. Rising 12th grade students may complete a Modified Schedule Permission Form (due May 31st) to request a free 7th period. If the free period conflicts with any graduation requirements, the request may not be granted. Any concerns or disputes must be addressed with administration.
