hyperkalemia
therapy (> 6.5 w/wo cardiotoxicity)
10% Ca gluconate 10 mL over 2-3 min
10 units regular insulin f/b 50 mL D5
furosemide 40 mg Q12H w/wo NS
patiromer 8.4 gm daily or zirconium cyclosilicate 10 gm TID 48h
albuterol 10-20 mg nebulized QID
Magnesium and Phosphorus Disorders
volume tolerance & fluid responsiveness
definitions:
volume tolerance: IVF resuscitation will cause no harm to the patient
volume responsiveness: cardiac output and tissue perfusion will improve after IVF resuscitation
modalities
pulse pressure variation (PPV)
a PPV of > 13-15% is associated w/volume responsiveness
stroke volume variation (SVV)
an SVV of > 10% is associated w/volume responsiveness
limitations: the patient must have all of the following
mechanically ventilated
tidal volume ≥ 8 mL/kg
normal sinus rhythm
no spontaneous breaths
