To facilitate the proper management of student dismissal, you must complete the Dismissal Choice Form. For the safety of all students, it is crucial that we receive your completed form by August 27th. This will allow us to finalize our dismissal procedures and ensure that every child is safely accounted for.The form allows you to indicate your preferred dismissal option for your child from the following choices:
- Walker Door
- Parent Pick-Up Line
- School Bus
- After-School Care Program
For all reimbursement requests, please download & complete the fillable PDF. Submit via email to NorthwestPTO@gmail.com You must include scanned or digital receipts. Alternatively, you can print & deliver completed forms with receipts to the main office. IMPORTANT NOTE: Only approved spending will be reimbursed - please consult with the PTO board before spending.
