Complete all applicable information, including your phone number and email. Be sure to select the reason for completing the form and include the nature of the qualifying event (QE)*, if applicable:
Add a new Qualified Beneficiary (QB) not linked to existing coverage through the MWTA Benefit Trust.
For new eligible MWTA or MWEA members:
Choose dental and/or vision insurance and coverage level.
If needed, provide dependent information for:
Spouse
Dependent Child(ren)
Update the enrollment status of a Qualified Beneficiary (QB) tied to existing coverage through the MWTA Benefit Trust.
Add or remove a Qualified Beneficiary (QB) due to a Qualifying Event (QE)*:
Specify the Qualifying Event (QE)* date and details in Section 2.
Notify the MWTA Benefit Trust of COBRA eligibility resulting from a Qualifying Event (QE)* causing loss of coverage.
Provide the following for each individual claiming COBRA eligibility:
Full name, address, email, and phone number.

Q: What is the Monroe-Woodbury Teachers Association Benefit Trust (MWTABT) ?
A: The MWTABT is the administrator of your dental and vision plans. The collective bargaining agreement settled in June of 2023 included a per member contribution from the district to the newly created MWTABT. With that contribution the MWTABT chooses and procures dental and vision plans for the members of the MWTA & MWEA.
Q: Does the MWTABT have anything to do with my health insurance?
A: NO.
Q: What if my dental or vision provider is not a part of the plan but would like to join?
A: Please contact Carmen Nardone (cnardone@mw.k12.ny.us) with the name and address of the dental or vision provider. That information will be forwarded to our insurance broker who can have METLIFE reach out to your provider.
Q: What determines if my employment at MWCSD makes me eligible for dental and optical benefits ?
A: Employees must work more than part-time in order to receive dental and optical benefits. A good rule of thumb is that if you qualify for health insurance with the MWCSD then you will also qualify for MWTABT optical and dental benefits.
Q: If I am a new hire with a job description covered under the MWTA or MWEA collective bargaining agreement and I am benefit eligible, how do I obtain dental and optical coverage ?
A: Please fill out the METLIFE enrollment form. Be sure to choose the enrollment form for your bargaining unit.
Q: I am eligible for a health insurance buyout. Can I still get the dental/optical benefits from the MWTABT?
A: YES
Q: My spouse and I both work for Monroe-Woodbury. Should we both have a plan and, if so, which plan should we choose?
A: Both you and your spouse should enroll in separate family plans, naming each other as dependents. Additionally, if you have children, both you and your spouse should name those children as dependents on each plan. This way together you double your benefit as you are both MWTABT members and entitled separately to family plans.
Q: If I worked for the MWCSD prior to the creation of the MWTABT and I am married to another member do I need to enroll in a family plan ?
A: NO, you and your spouse were identified prior to the change over and now both have family plans as members of the MWTABT.
