Please complete the form in as much detail as possible, including evidence of the violation and witnesses. Once filed, an investigator will reach out within 3 business days to schedule your statement recording.
*If you are reporting an Officer for a violation of the MidwestRP Professional Expectations Policy (i.e. racism, sexism, harassment), dual clanning policy, or if the Officer you're reporting is the Chief of Police or Assistant Chief of Police, please file the MidwestRP Internal Affairs Complaint Form.
Any LSPD Officer who has received disciplinary action, regardless if PDIA was involved, may request to appeal the disciplinary action against them.
To request an appeal, write a formal letter requesting the appeal. Include why you feel the disciplinary action should be appealed. Send the letter directly to the Chief of Police, Ty Fox. The letter must be sent to Chief Fox within 7 days of receiving the disciplinary action.
