The CNUSD Athletic Clearance process has moved online. Please read the instructions on the new clearance process below.
To complete your athletic clearance:
Visit the Online Clearance Form here: https://permission.click/4K6Z7/us
Once all required items are completed on this online site, please print the actual Physical Form (this is the form you will take to your health care provider) & Emergency Form.
Take the Physical Form to your health care provider & have it completed by an MD, DO, PA or NP.
Final clearance will be granted once the completed Emergency Form & Physical Form have been submitted to the Athletics Office for your school site.
You will receive a signed copy of the Emergency Form from the Athletics Office. This copy of the Emergency Form is your receipt. Present this Form to your coach when you attend the first practice or tryout.
Due to the school closures, please e-mail the completed copies of your Emergency Form & Physical Form to the following emails for each school site:

We will be meeting up in front of school in our cohorts/pods. After you are cleared, you will head over to the park for practice. Please make sure your ride waits for you to be cleared (usually less than 5-10 minutes), and takes you over to the park where we will warm-up, do drills and workout in our training pods.

Reminder: Do not come to practice if you are sick. Do not come to practice if you are not currently cleared. Do not come to practice if you have come in contact with someone who has or may have COVID. Please notify Coach Cummings if you have questions. Thanks.
Please: Maintain social distancing as best as you can and wear a mask along with washing hands frequently and showering after practice.
The athletics directors and coaches in the league believe we will be able to have inter-league competition in February and March. The healthiest athletes will be entered into league meets, and entries are completely up to coaches' discretion this season. If an athlete is not regularly attending practices, there is little chance they would race.
Campus courses have been mapped at three different sites, including Roosevelt, and Riverside City Course is looking like a potential site for competition. If a competition does take place, no spectators would be allowed on site. Only cleared athletes, coaches, and officials.
AD's are working on a pathway for safe competition. Let's do our part to stay healthy and safe.
Check-in is at school every day but 1/18/21. We will check-in at the park that day with temperature scanners.
*For training days, please bring your own mat or beach towel, and resistance bands if you have them. Thanks.
*Also, we advise you to not share equipment, and we do not recommend carpooling.
