IMPORTANT: Your Athletic Clearance Must Run through your season of sport or you will be required to get a new one before being allowed on the team. You will not be allowed in an Athletic Period without a Physical that is good for the school year. 

Steps to completing your athletic clearance

1.  Print Physical Form & Emergency Form located next to these instructions Physical Form, Emergency Form. 

2. Take the Physical Form to your health care provider & have it completed by an MD, DO, NP, or PA medical professional prior to filling out the online form. Also complete the Emergency Form.

3. Visit the link below and complete the online questionnaire and upload your  Physical Form & Emergency Form 

4. Once completed you will receive confirmation email. Please forward that email to erhsathletics@cnusd.k12.ca.us

• Please attach your Emergency Form & Physical Form to the email in PDF format.

CIF mandates all athletes must be covered by insurance in order to participate in any try-outs, practices or games. If you do not have insurance coverage you may purchase coverage through Meyers, Stephens & Toohey.  

• Parent needs to complete & sign:

  • Emergency/Transportation Form – A copy will be provided to you to give to coach in order to participate (it is your receipt to participate). 

• Physical Examination Evaluation:

  • Must be completed by a licensed Medical Doctor (MD) or Doctor of Osteopathic (DO), CNUSD now accepts Physician Assistants (PA), or Nurse Practitioners (RNP). Make sure the Physical Form is completed with the doctors stamp and date.

• New Transfer Students:

  • Must complete CIF 207/510 paperwork & return to the Athletic Office as soon as possible.

  • Must bring in last report card from former school

Do not give Athletic Clearance documents to a coach or leave on the desk.  Clearance must be uploaded to Online system or handed to Mrs. Schnakenberg (Athletic Secretary) ONLY.
