• Student Account Disbursement Form

  • Use this form when you would like to use your student's account to pay for something, such as trip payments, band camp, supplies, etc.

• Fees can easily and safely be paid online via CutTime.  

• In the event that you are unable to pay via CutTime, you may pay via personal check made out to MCHS Band Boosters.  

• Payments and forms can be turned in via the Black Box in the Band Room.  The Black Box is located on the wall to the left of the door leading to the main part of the school. Be sure your STUDENT'S NAME is clearly indicated on any forms or payments you turn in.

Printable Marching Band schedule

This schedule changes frequently and the printable version is not always up to date.  For the most current Marching Band schedule, please see the Google calendar. 

These forms are included for your convenience, however responses for these go directly to the Band Directors.  Any questions or concerns regarding these forms should be directed to Mr. Adelmann or Mr. Hamilton.

• Release to Parent Form

  • This form can be accessed by your student in the the Google Classroom for Band. They must log in with their MCHS student email to access this form.

  • Parents choosing to take their child(ren) home following an off-campus band event must submit a release request at least one (1) day in advance of the event. The student and their parent/guardian must check out with the bus chaperone before leaving. Students can only be released to their OWN parent/guardian.  

  • FORMS SUBMITTED LESS THAN 24 HOURS BEFORE THE EVENT MAY NOT BE PROCESSED. 

• Last Minute Absence Form

  • This form can be accessed by your student in the the Google Classroom for Band. They must log in with their MCHS student email to access this form.

  • This form should be used for last-minute absences from practices or performances. 

  • Filling out this form is necessary in order for your illness/injury/family emergency absence to be excused.

  • Filling out this form for any other type of absence (work, etc) does NOT mean that absence is excused - only that you have effectively communicated it to us. Absences for unexcused reasons (work, it's your birthday, etc) will still be unexcused.
