If your family is willing to donate this year, please print, complete and mail this form to the school with your check made out to Sunset Ridge PTA. Donations can also be made via Venmo (@SSRPTA) using the QR code below. Please add a note with "Annual PTA Donation," your student(s) name and grade.

We also welcome you to join our PTA! Please become an active member of our PTA. For just $6 you will become a member of the Wisconsin PTA, be invited to our meetings and have voting rights to decide what we purchase for Sunset Ridge with our fundraising dollars.
