All of the data that I gathered from the Home Delivery program clients was collected through phone surveys. I created a question form using Google Forms, and I then called the participants, asked them the questions, and filled out their answers online. 

Most of my questions were rating scales from 1-5. This made the data easier to analyze and the questions easier for the clients to understand. I also used a few short-answer questions to record more personalized opinions.

All of the graphs used here were created in Google Forms.
