This page contains links to downloadable documents and forms that are helpful to LW EVSTL captains. 

• • Food Signup Sheet -- This form can be used to help the captains assign helpers (volunteers?) for the preparation of food for the EVSTL matches.

A Score Tally Sheet has been created. 

• Forms A thru D  have the seven-point tiebreakers tallies on Page 2.  

• Form E is a new (2022) Score Tally Sheet:

• 1. 1. Page 1 has the Tally Sheet and Tiebreaker tally for Set 1.

     2. Page 2 has the Tally Sheet and Tiebreaker tally for Set 2 and also the Set Tiebreaker tally. 

• Form F is like Form E but two pieces of paper rather than one ,

Note: Be sure to set the Print dialog box to LANDSCAPE.

Five (5) forms are available:

• • Form A -- This form contains Page 1 and Page 2 on one side of an 8 1/2 x 11 sheet of paper.

  • Form B -- This form contains two copies of Page 1 on one side of an 8 1/2 x 11 sheet of paper.

  • Form C -- This form contains two copies of Page 2 on one side of an 8 1/2 x 11 sheet of paper.

  • Form D -- For those of you who have two-sided printing capability, this form is a combination of Form B and Form C.  When you print this form out you have two copies of Page 1 on one side of the printout and two copies of Page 2 on the other side of the printout.  Note: Be sure to set the Print dialog box to flip on the SHORT side of the page.

  • Form E -- This form is a new (2022) Score Tally Sheet. It contains Page 1 and Page 2 on one side of an 8.5x11 sheet of paper.

  • Form F --  Like Form E but has Page 1 on on piece of 8.5x11 paper, and page 2 on another piece of paper.
