Due to the high volume of requests at the DILG Central Office, we recommend applying early in case of needed revisions.
All documents must be in pdf format with a file size not exceeding 25 MB.
The Department shall not be precluded from requesting other additional documents, as may be necessary, for the approval of travel authority request.
Template for Requirement 1. A duly notarized affidavit attesting that no administrative charge or criminal case has been filed or is pending against the applicant with the ID card being presented

For Punong Barangays, the Certification on Money and Property Accountabilities shall be signed by the Barangay Treasurer and the Barangay Secretary.
For Barangay Treasurers, Barangay Secretaries, and Sangguniang Kabataan (SK) Officials, the Certification on Money and Property Accountabilities shall be signed by the Barangay Treasurer and Barangay Secretary, and duly noted by the Punong Barangay.

For Sangguniang Members, the approving authority for the Leave of Absence shall be the Vice Governor or the Vice Mayor.

When one trip includes travels to multiple countries, prepare a simple itinerary of travel. You will be asked to encode the specific dates of your visit to each country.

What does the STATUS mean?
The following are the processing levels to help you track the progress of your application. Your application will need to go through several levels before you receive the Foreign Travel Authority in your email:
1. Receiving

4. Endorsement
5. Approval
In case there are discrepancies or issues in your initially submitted documents, email the revised documents to fta@dilg.gov.ph with the email subject as the FTA number and Name of applicant. (Follow specific instructions indicated in your received email, if applicable.)
All applications submitted through the FTA Portal that are incomplete or not properly accomplished shall be forfeited and automatically deleted from the system two (2) days after notification of the missing document(s), should the applicant fail to provide the prerequisites.
DO NOT submit a new application in the web portal. Applicants are advised to wait for full remarks and instructions in an email from the DILG Central office from no_reply@dilg.gov.ph or fta@dilg.gov.ph. Read the instructions carefully.
