 Yes, you can get a Yellow DOT packet for every vehicle you drive.

Yes, you should complete the medical information form for any other driver(s) or passenger of your vehicle, especially if they have a specific medical need (i.e. autistic, difficulty communicating in stressful situations, medical allergy, diabetes, heart problems).

The Yellow DOT decal should be placed on the driver’s side rear window of the vehicle and the brochure/medical information sheet in the glove box. 

For motorcycles - Place the decal on the "triple tree" of the motorcycle and put the packet in the saddle bag/compartment.

The photo is crucial to the success of the program. Yellow DOT is about saving those precious seconds immediately following a serious incident. Having a photo identifying to whom the medical form belongs will save time and possibly your life. Please make sure a photo(s) is included with each medical information form.

Only you have access to the information in your glove compartment. The information is not kept by any agency or another person.

No, do not include social security number, credit card information or any other sensitive information.

No, the program is FREE!

ABSOLUTELY! We ask that you email katc@louisville.edu at least 2 weeks in advance as these materials are made in house. 

We estimate that the Yellow Dot materials cost approximately $1 each to create and mail so please be mindful of the number you are requesting. 

TIP: We recommend having an event or segment of an event where participants actually fill out the brochure/medical form and then everyone goes to their vehicles and adheres the decal on the rear windshield.
