Wrestling Eligibility

All of the following must be complete in order for an athlete to participate in Wrestling at James Madison High School

• • • The VHSL requires this form—and will accept no other notes or forms—to allow wrestlers to participate if they are actively treating a skin lesion, such as ringworm, impetigo, staph, etc. Wrestlers should give a completed copy to coaches as well as have their own copy with them at all wrestling events throughout their treatment. 

    • Additionally, wrestlers who suffer from excessively dry skin or eczema are encouraged to complete the form as well to avoid any misunderstandings with officials.
