Download and fill out Form SS-5 (Application for a Social Security Card) from the Social Security Administration website.
You must provide at least two original documents to prove your age, identity, and citizenship.
Proof of Age: Birth certificate
Proof of Identity: A U.S. driver's license, state-issued ID, or U.S. passport
Proof of Citizenship/Status: Birth certificate or passport. Non-U.S. citizens need documentation of lawful work authorization or a valid non-work reason for the card.
Go to a local Social Security office with your completed application and original documents to complete your application and interview.
For a Replacement Social Security Card
For replacement cards, you may be able to apply online, by mail, or in person.
Online: You can request a replacement card online in participating states by creating or logging in to a My Social Security account.
In Person: You can also begin your application online and then schedule an appointment to visit a local Social Security office or Card Center to finish the process.
Documents: Only original or certified documents are accepted.
Appointments: Making an appointment can reduce your wait time when visiting an office.
Limits on Replacements: There are limits on the number of replacement Social Security cards you can receive per year and in your lifetime.
