Losing important documents like your birth certificate, Social Security card, or state ID can be stressful—but you're not alone, and help is available. Follow these steps to begin the recovery process and protect your identity.
Take a moment to identify exactly which documents are lost. Common vital documents include:
Birth certificate
Social Security card
State-issued ID or driver’s license
Passport
Immigration documents
Marriage or divorce certificates
Make a list of what’s missing so you can take targeted action.
IRC may have electronic copies of your documents on file.
Your caseworker, or other IRC staff member, can help guide you on what to do next.
If your documents were stolen, report the theft to:
Local police: File a report to protect yourself from identity theft.
Federal agencies: For lost passports or immigration documents, notify the U.S. Department of State or USCIS.
Try and re-trace your steps and contact local businesses where you may have lost the card.
If you lost your documents on a bus or light rail, you can file a Lost and Found report.
Each document has a different process for replacement. Here’s how to get started:
Birth Certificate
Contact the vital records office in the state where you were born.
You may need to provide ID and pay a small fee.
Social Security Card
Visit SSA.gov to request a replacement.
You can request a free replacement card by filling out the online questionnaire
After you submit the request, call +1 800-772-1213 to make an in person appointment.
You’ll need proof of identity and possibly citizenship.
State ID or Driver’s License
Visit Department of Motor Vehicles (DMV) website.
Schedule an appointment at a DMV near you
Select "Renew Colorado Driver License/ID/Permit"
You may need to appear in person and provide supporting documents.
Passport
Report the loss to the U.S. Department of State.
Complete Form DS-64 and apply for a replacement using Form DS-11.
Immigration Documents (EAD, Green Card)
Contact USCIS for replacement forms like I-90 (Green Card) or I-102 (Arrival/Departure Record).
If your EAD is lost, stolen or destroyed, you may request a replacement EAD by filing a new Form I-765 and filing fee.
