The loss book that is on loan must be reported to the section where the book was charge out.

 A lost look must either be (a) paid according to the value  determined by the library plus processing fee of P75.00  (b) replaced by the copy of same book (c) replaced with a different title upon the approval of the librarian. The amount paid for a lost and found book may be refund within the immediate succeeding semester. After this period, no refund is allowed. 

Library materials damaged by a user or borrower beyond normal usage must be replaced with the same title with additional cost or to be paid as determined by the librarian
