Welcome to Heartland Charter School's Adult Transition Program (ATP), designed specifically for students who have completed 12th grade, are 18 years or older, and have earned their Certificate of Completion. Our ATP caters to students who require more explicit instruction in crucial areas like job coaching, functional academics, independent living skills, and self-advocacy.
Through our virtual instruction, we continue to empower students in these domains and complement their learning with extension activities in the community. Moreover, we understand that agency linkages are essential for a successful transition to adult life. Our dedicated ATP case manager collaborates closely with regional centers, the Department of Rehabilitation (DOR), and other relevant local agencies to provide comprehensive support during this critical phase. Some of the services we facilitate include:
Connecting students with job coaches through DOR for personalized career guidance.
Assisting students in their 21st and final year of school to find appropriate adult programs and living facilities through coordination with the regional center.
Linking students with the Disabled Students Programs and Services (DSPS) to ensure continued education opportunities at local community colleges.
As part of our holistic approach, students will also participate in Life Skills classes to support their homeschooling, including a Community Experiences course. Other Life Skills courses will be supported through parent-guided curriculum such as Unique, PCI, Conover, and Course Outlines. Furthermore, students enrolled in ATP will continue to receive vital related services from their IEP, such as Speech, Occupational Therapy (OT), Educationally Related Mental Health Services (ERMHS), Counseling, and Physical Therapy (PT).
