Assisted Living Facilities: These facilities provide nursing and rehabilitation services and addition to personal care, 24-hour supervision, social services, and recreational activities. These facilities are for people who do not function independently but do not require skilled nursing care.
Skilled Nursing Facilities: In nursing homes at a higher end of the institutional continuum. The patients are usually transferred from a hospital to the SNF after an acute episode.
Subacute Care Facilities: A subacute care facility is a place that offers inpatient care and rehabilitation for patients who need more intensive care than a nursing home but are not critical enough for an acute hospital ward.
Continuing Care Retirement Communities: A facility that is only for senior living and provides housing, health care, and social services in one location.
Home Health Care: Home health care is a range of skilled medical services that are provided in a patient's home to treat or manage an illness, injury, or medical condition. Home health care can include nursing care, physical therapy, occupational therapy, speech-language therapy, and medical social services.
