Guideline

Upper case/lower case, with or without colons, follow Table of Contents even if there is a discrepancy with the first page of the chapter

If Table of Contents uses "Song" "Act" "Battle" use those words instead of "Chapter" 

When the last episode is named differently such as "Final chapter" instead of "Chapter 60", please use the chapter title as in Table of Contents

Include the subtitles as in Table of Contents

If the chapter number is spelled out in Table of Contents and chapter page, then for readability, the number could be in numerical figure

When the chapter name format changes within the series, follow Table of Contents of Vol. 1

However, name chapters according to Table of Content when it's changed with purpose and numbered independently.

Exceptions 

When the first page of the chapter name or number is correct (even when checked against the Japanese original), which is a Table of Contents mistake by the English editorial

When a chapter consists different mini stories with a title for each one, you can call name the chapter "Chapter 1"

If the name of a chapter in the Table of Contents is different from the name on the chapter cover, check the content and refer to other sources to determine the correct name.

If there is no Table of Contents, follow the chapter cover. 

If the format of chapters changes in later chapters, still follow the format of the first chapter.
