The sample and pledge are the same. Visit each storeshoue during the opening hours of the storehouse and fill out the pledges. After that our staff will confirm the luggage and pledge(own). You do not have to print it out before visit to each storehouse. 

(Instead, you should bring the pledge of the storehouse storage(deputy), after fill all the form. And then please summit to the staff.)

We are not accept the pledge of the storehouse storage(deputy) during the spring and fall semester.
