Information/First Visit

Information regarding your first visit, is listed below.

What you need to know:

Where to park: My practice is in the gym of Glen Raven Baptist Church. There are three buildings on the property. The gym is the building that has a metal roof. Park in the parking lot in front of the glass doors.

What to bring: Please remember to bring any insurance information, a medication list, your copay, and the Client Intake Form and Release of Information Document (below under forms). We would like to use every minute for therapy; if you bring the information in ahead of time, we can begin therapy right away!

Policies:

Payment is due at the time of service.

Fitzgerald Counseling requires a 24 hour cancellation of appointments except in the case of emergencies. Cancellations outside of this time frame will be charged a $25 fee.
