Long-term care programs provide any service or support a person may need as a result of getting older or having a disability that limits their ability to do the things that are part of their daily routine.
Fond du Lac County Specialized Children’s Services:
Fond du Lac County Specialized Children’s Services coordinates and manages an array of comprehensive services for eligible children with a severe developmental disability, physical disability, autism, or emotional disability. Services are individually designed to help support the child and family and to promote health, safety, participation and independence in the home and community.
Eligibility for Children’s Long Term Support Services is based on the Children’s Long Term Support Functional Screen results, an assessed need for long term support services, and Medical Assistance eligibility. Fees may be applied for services based upon the family’s ability to pay.

Which Long-Term Care Program is Best for Me? Family Care, Partnership/PACE or IRIS
The Family Care Program is a Medicaid-based program that provides support to individuals who meet certain eligibility criteria. There are three managed care organizations (MCOs) in our county that administer the program: Community Care, Inc., Lakeland Care, Inc., and Inclusa. There is a care manager and nurse assigned to each member in the program that assesses the individual's needs and create a care plan to meet those needs. This may include services like in-home care, medication management, transportation, and possible residential placement in a facility if needed. The Family Care Partnership Program is a Medicaid-based program that provides support to individuals who meet certain eligibility criteria. There is one managed care organization (MCO) in our county that administer the program: My Choice Wisconsin. There is a care manager, nurse care manager, nurse practitioner, physician, and case aid assigned to each member in the program, that assesses the individual’s needs and create a create a care plan to meet those needs. They may include services like in-home care, medication management, transportation, possible residential placement in a facility, and etc. The program covers both long-term care services and health care. IRIS is a Medicaid-based program that provides support to individuals who meet certain eligibility criteria. There are five consulting agencies in our county that administer the program: MILC (Midstate Independent Living Choices), TMG (The Management Group), Advocates 4U, Connections, and First Person Care Consultants. The consultant can provide the individual with support on creating their own care plan using a budget of money that is based off the individual's needs. They may pay for things like in-home care, medication management, transportation, and possible residential placement in a facility if needed. Choosing an IRIS Fiscal Employment Agency (FEA)
In addition to choosing an IRIS ICA, a FEA will need to be chosen. The FEA will support the individual in IRIS by processing paperwork to hire workers, perform background checks, etc. This is similar to a payroll department. Click here to read more about these.
