explanation of fees
Evaluations: Established fees for all types of evaluations include the testing, conference, and written report. In order to schedule a testing date, a deposit is required. The balance of the evaluation fee is due on the morning of testing.
Consultations: Consultations are scheduled at an hourly rate based on the specific request. Payment is due at the time of the consultation. No deposit is required.
Payment: While we do not submit health insurance, we do provide medically coded documentation for clients who wish to file for reimbursement with their own insurance company.
Payment Method: Acceptable methods of payment include check, ATM/Debit cards (VISA, MasterCard), Health Savings Account (HSA), and credit cards (VISA, MasterCard, Discover, American Express). Please make checks payable to Shelley B. Benton, Ph.D.
Cancellation and Missed Appointment Policy:
The deposit required for evaluations guarantees the client's testing date. If a client cancels an evaluation appointment, the deposit is forfeited. However, if a client reschedules an evaluation appointment, the deposit will not be forfeited, but a fee may be charged in certain circumstances. Consultations need at least 24 hours notice for cancellation or clients will be charged a cancellation fee.
