A referral is required to see the urologist.
Wait times are as follows ( subject to change)
Non-Urgent & Routine concerns : 12-18 months wait time
Vasectomy: 18-24 + months wait time
*Please note there are only 2 urologists in Chatham Kent so wait times can be long at times*
Dr. Gabrigna Berto does not accept pediatric referrals.
If you need to cancel an appointment you may call our office and leave a voicemail.
Please try to give at least 24hrs notice.
There is a fee for missed appointments, payable prior to rebooking.
100.00 for no show consultation/ procedure at hospital
50.00 for no show follow up appointment

INITIAL CONSULTATION APPOINTMENT INFORMATION:
Prior to your consultation appointment, please do the following:
Complete all requested lab work complete at least 1 week prior to appointment
Bring a list of your medications.
Valid Health Card is required*
An itemized list of previous medical diagnoses and surgical procedures (if applicable) is also helpful.
If this is a follow up appointment, please inform of us of any significant interval change in your health since you were last seen

AMBULATORY CARE APPOINTMENT?
Dr. Gabrigna Berto also sees patients at
CKHA Ambulatory Care.
Located at 80 Grand Ave West Chatham
Entrance on the back side of the hospital ( Off Louise Street)
If you have an appointment there- Please register 15-30 minutes before your appointment at the
Ambulatory Care Registration Desk
A Uroflow test may be completed during your appointment.
Patients urinate into a special toilet or funnel connected to a computerized machine.
It is best performed with a full bladder.
SCHEDULED FOR SURGERY?
If you are scheduled for a surgery
this will take place at Chatham Kent Health Alliance
Register at front main doors and and then they will direct you to Day Surgery
You will have to register 1.5hrs before your scheduled
surgery time.
Please follow any pre-op instructions you were given.
Suspected UTI: Please contact your Primary Care Provider (PCP). They can facilitate a urine sample to be sent to the lab for testing.
If you do not have a primary care provider, a pharmacist may be able to treat.
Catheter Issues: * If you have a home care agency (e.g., Bayshore or VON), contact them immediately for assessment.
Otherwise, contact your Primary Care Provider.
Emergencies: If you are experiencing a medical emergency, please proceed to your local Emergency Room immediately.
Contact your pharmacy to initiate a refill request via fax:
Non-Urgent: The pharmacy should fax the request to Dr. Berto’s office.
Urgent: If Dr. Berto is away, have the pharmacy fax the request to your Primary Care Provider.
If you are experiencing concerns or side effects with medication please speak to your pharmacist or primary care provider.
If you need to confirm or cancel an appointment with Dr. Gabrigna Berto- A voicemail can be left and we will call you back to reschedule if needed.
New Referrals: All referrals are triaged by the urologist. You will be contacted with a date as soon as one becomes available.
Wait Times: For non-urgent concerns, current wait times are approximately 12–18 months.
Follow-up: A referral acknowledgement is sent to your referring physician/PCP, which includes estimated wait times and your consultation date.
Diagnostic Imaging Status: If you are waiting for a call regarding imaging(CT, MRI,Ultrasound), please contact the Chatham Kent Health Alliance – Patient Appointment Office at 519-437-6089.
