OFFICE OPEN BY APPOINTMENT ONLY, PLEASE CALL 519-598-9207
After we receive a referral from your family physician or other healthcare provider, we will contact you with appointment details. Due to the high volume of referrals that we receive, please be aware that it may take some time to process the referral. We appreciate your patience.
All referrals are triaged according to the level of clinical urgency. We attempt to see everyone as soon as possible.
We will contact you directly with an appointment date once available. A notice will also be sent to the referring doctor/family doctor with the appointment details.
If you no longer require your appointment or need to re-schedule please call our office and we will be happy to help you, you may leave a voicemail.
Please try to cancel or reschedule as soon as possible to allow us to contact other patients who may be waiting for an appointment
If appointments are cancelled without at least 1 business day’s notice, except in exceptional circumstances, a $100 no show fee may be applied.
This is policy to ensure that we maximize our ability to accommodate all patients as no show appointments contribute to our wait lists.
Payable in cash only as we do not have a debit or credit machine available.
Why are no-show fees billed?
With many patients on our waiting list, we hope that everyone has access to an appointment in a timely fashion, and a no show spot could have been offered to someone else on the waiting list.
We do ask that the no show fee be paid before we book another appointment , as we have had cases of multiple no shows for the same patients.
Ask your Pharmacist to send a Refill Request to our office and the doctor will review and forward a response to pharmacy as soon as able. Please try to request the refill 1-2 weeks before your medication runs out.
Unfortunately, we cannot provide emergency care over the phone or in the office.
If you are experiencing an emergency, please call 911 and/or proceed to your nearest emergency department to be assessed
The doctor ordered an Ultrasound, MRI or CT for me but I have not received a call for an appointment date yet?
