Details:

Each week two Deacons are responsible for reaching out to members of our congregation who are 80+ years old and living in an assisted living home, skilled nursing facility or at home.  Yolanda Bragg will send you an email with the names of the members you are to follow-up with, the names of the facility, addresses and the person's room number.  You will have a week to follow-up with your assigned members.  You should call first and then schedule a visit.  If you are unable to connect, you could send them a card.  

It is important that you find another Deacon willing to substitute for you if you are unable to participate on the week in which you have been assigned. Everyone is asked to sign up a few times per year. 

After your call/visit, please email Alison Messick-Watkins to let her know you made the connections and give her any updates so she knows how to follow up when needed.

In some cases, you may need to reach out to family members to let them know that you called or sent a card to their loved ones. If we have family information available it will be provided. This is especially important for members with dementia.

From Joe Olivo:  "I send these cards to those MPPC members who I call when doing the visitation calls. I have found that this has been helpful to let individuals know that the church is thinking about them especially if you can’t reach the individual by telephone. It can be printed on 5x7 card stock. I generally get the card stock from Amazon or Office Depot. Yolanda also has MPPC card stock."
