The Tiered Fidelity Inventory (TFI) is a survey typically taken 3 times annually (2020-2021 year the district expectation is to take it twice as a part of our Leadership Academy) to assess the extent to which school personnel apply the core features of School Wide PBIS at all three tiers – either individually or collectively. Teams may take the TFI for a variety of purposes including:
An initial assessment to determine if they are using, or need, PBIS
A guide for implementation of Tier I, Tier II, and/or Tier III practices
An index of sustained PBIS implementation
A metric for identifying schools for recognition within their state implementation efforts

The Self-Assessment Survey (SAS) is an annual, multiple-response survey to help teams identify the staff perception of implementation status and improvement priority for school-wide, classroom, non-classroom, and individual student systems. This is a SAS Google Form created in the 2019-2020 school year that can be used by buildings for their own assessment and planning.

Students in grades 3 through 12 completed the Conditions for Learning Survey to assess safety, student engagement and the overall learning environment in each building. Due to statewide reporting issues, during the last CFL administration only surveys from students in grades 6 through 12 were included in the schools' accountability scores, while all surveys from students in grades 3 through 12 were included in the in-depth view. This data provides important information about student perception and serves as a foundation on which to build an action plan for improving the learning environment for all students. The information will also help to determine the level of support needed by schools, and the resources they may need to provide an optimal learning environment for all learners. This is a CFL Google Form that was created in the 2019-2020 School Year for buildings wanting to assess aspects of the CFL on their own.
The SDQ is a universal screener for social, emotional, and behavioral health. Students taking the assessment are scored on a scale from low, moderate, high, or very high risk. Students scoring in the high or very high risk level will have interventions put in place to best meet their needs. Only schools and staff trained can provide this screener.

Different from the SDQ which is a screener for social and emotional well being for selected schools only, the Davenport Schools Student Needs Assessment is for all students in grades 3-12. It helps us to identify students and families who may need additional support with more tangible needs. Students are able to tell us if they have enough food, clothing, and stable housing. They also have the opportunity to inform us if they have needs concerning vision/dental health, internet, holiday giving, counseling services, etc. There is a needs assessment for students in grades 7-12 and a similar assessment catered to students in grades 3-6. The deadline to complete the screener was October 10, 2020.
