Following the steps and procedures listed below helps ensure students' records are submitted by the colleges' application deadlines. Students should follow CHS Transcript Request deadlines to ensure records are sent on time.
Click the link below to download the Consent Form. The Consent form gives your counselor permission to send your transcript and other supporting documentation to schools electronically, regardless of the option marked. It is a required document for all seniors, regardless of post-grad plans. By indicating "YES", you are waiving your rights to view/see your letter of recommendation. Know that counselors will support your application in an accurate, authentic way using a positive tone. By indicating "NO", you’re choosing not to waive your right, which allows you access to your letter of recommendation written by FCPS staff. However, colleges ask us to disclose that you did not waive your right and may consider that in their evaluation.
Be sure you have included:
Name (Last, First), Student ID #, Preferred Email Address & Counselor Name
Application Deadline Date, Admission Type (Regular, Early Decision, Early Action, etc.), Application Method, Name of College, City and State/County of College
Student and Parent/Guardian Signatures
It is imperative that you PLAN AHEAD and follow the dates listed. Late requests risk not being sent by the deadline.
