Steps to Submit a Transcript Request
Following the steps and procedures listed below help ensure students' records are submitted by the colleges' application deadlines. Students should follow CHS Transcript Request deadlines to ensure records are sent on time.
Step 1 - Make sure you have a Transcript Release Form on file.
Step 2 - Fill out the Transcript Request Form completely.
Step 3 - Bring your completed request form to Mrs. Marsengill in the Transcript Office, located in Student Services.

Download and save the form as a PDF.
This form must be on file with the Transcript Office in order for CHS to release official transcripts to the colleges and scholarship organizations you request.
Many seniors already submitted their forms in the Spring. This form only needs to be submitted once.
STEP 2 - Fill out the Transcript Form Completely

Be sure you have included:
Name (Last, First), Student ID #, Preferred Email Address & Counselor Name
Application Deadline Date, Admission Type (Regular, Early Decision, Early Action, etc.), Application Method, Name of College, City and State/County of College
Student and Parent/Guardian Signatures

Notify the Transcript Office and your counselor directly if you change what application you are using after you submit your requests.
STEP 3 - Bring your completed form Mrs. Marsengill in the Transcript Office, located in Student Services.
It is imperative that you PLAN AHEAD and follow the dates listed. Late requests risk not being sent by the deadline.
