The Classic Triad
The majority of patients with Budd-Chiari syndrome present with the classic triad of symptoms [2].
This triad often presents rapidly, and these features reflect the underlying disruption of hepatic blood flow and increased pressure within the liver.
Abdominal pain, typically localized to the right upper quadrant, is one of the earliest and most common symptoms. It occurs due to hepatic congestion, where blood accumulates within the liver because it cannot effectively drain through the hepatic veins. This leads to stretching of the Glisson’s capsule, a fibrous covering of the liver that is highly sensitive to pain. The degree of pain can vary depending on how rapidly the obstruction develops, with acute cases often presenting more severely.
Ascites refers to the abnormal accumulation of fluid within the peritoneal cavity and is a hallmark feature of Budd-Chiari syndrome. It develops as a result of increased sinusoidal and portal venous pressure, which forces fluid out of the vasculature* and into the abdominal space. In addition, reduced liver function can impair the synthesis of proteins such as albumin, further contributing to fluid leakage. Ascites may develop rapidly in acute cases or progressively in chronic disease.
Hepatomegaly, or enlargement of the liver, occurs due to venous congestion and blood pooling within the hepatic tissue. As outflow is obstructed, the liver becomes swollen and may be tender on physical examination. Over time, persistent congestion can lead to hepatocellular injury and structural changes within the liver, which may alter its size and consistency depending on the stage of the disease.
Progression of Symptoms
The clinical features of Budd-Chiari syndrome evolve over time depending on the rate and duration of hepatic venous obstruction. In early stages, symptoms are primarily driven by acute hepatic congestion and increased sinusoidal pressure, whereas later stages reflect the development of portal hypertension and progressive liver dysfunction. Understanding this progression helps link the underlying pathophysiology to the changing clinical presentation observed in patients.
Right Upper Quadrant Abdominal Pain 7
This is often one of the earliest presenting symptoms and results from rapid hepatic congestion. As blood accumulates within the liver due to impaired venous outflow, the liver capsule becomes stretched, producing localized pain in the right upper quadrant. The pain may be sudden and severe in acute cases, reflecting the abrupt onset of obstruction.
Mild Hepatomegaly 2,7
In the early stages, the liver becomes enlarged due to venous congestion and blood pooling. This enlargement is typically mild to moderate and may be accompanied by tenderness on physical examination. The increase in liver size reflects the immediate hemodynamic effects of impaired outflow rather than long-term structural changes.
Nausea and Fatigue 7
These nonspecific symptoms arise as a result of early hepatic dysfunction and systemic effects of congestion. Reduced liver perfusion* and metabolic disruption can contribute to a general sense of malaise, while the accumulation of metabolic byproducts may lead to nausea and decreased appetite.
Rapid Development of Ascites 6
Ascites may develop quickly in acute or subacute cases due to sudden increases in sinusoidal pressure, which force fluid out of the hepatic vasculature into the peritoneal cavity. Because this process occurs rapidly, patients may notice noticeable abdominal distension over a short period of time.
Progressive Ascites 3,7
In chronic disease, ascites become more persistent and difficult to manage, reflecting sustained portal hypertension and ongoing fluid imbalance. In addition to increased hydrostatic pressure, impaired protein synthesis by the liver (e.g., reduced albumin) further promotes fluid accumulation.
Splenomegaly 6,7
Splenomegaly develops as a consequence of prolonged portal hypertension, which causes blood to back up into the splenic circulation. This leads to enlargement of the spleen and may be associated with hypersplenism, contributing to cytopenias*.
Jaundice 2,7
Jaundice reflects progressive hepatocellular dysfunction, where the liver is no longer able to effectively process and excrete bilirubin. This results in yellowing of the skin and sclera and typically indicates more advanced disease.
Variceal Bleeding (Due to Portal Hypertension) 6
As portal pressure increases, the body forms collateral vessels* to bypass the obstructed hepatic circulation. These vessels, particularly in the esophagus and stomach, are fragile and prone to rupture, leading to potentially life-threatening variceal bleeding*.
Signs of Liver Failure (Encephalopathy) 6
In advanced stages, severe liver dysfunction can lead to hepatic encephalopathy*further detail in section 4*, characterized by confusion, altered mental status, and impaired cognition. This occurs due to the accumulation of neurotoxic substances, such as ammonia, that are normally detoxified by the liver.

Severe or worsening abdominal pain
Sudden onset or worsening jaundice
Gastrointestinal bleeding (e.g., vomiting blood, black stools)
Confusion or altered mental status (hepatic encephalopathy)
