Chronic hepatic venous outflow obstruction leads to hepatic congestion, resulting in reduced blood flow and oxygen delivery to liver tissue. This causes hepatocellular injury* and loss, particularly in the centrilobular regions*. Over time, persistent congestion and cell damage promote the development of fibrosis. Fibrosis is characterized by the accumulation of extracellular matrix and scarring. As fibrosis progresses, it can lead to cirrhosis, marked by architectural distortion and impaired liver function, further disrupting hepatic blood flow and contributing to disease progression.
In Budd–Chiari syndrome, obstruction of hepatic venous outflow causes blood to accumulate within the liver, leading to increased sinusoidal pressure. This elevated pressure is transmitted backward into the portal venous system, resulting in portal hypertension. As portal pressure rises, collateral circulation* develops to bypass the obstruction, forming portosystemic shunts and varices*.
Clinically, this presents as ascites, splenomegaly, and variceal formation*. Persistent portal hypertension worsens fluid accumulation and increases the risk of life-threatening variceal bleeding*.
Building on liver fibrosis, continued hepatic injury and chronic congestion drive a sustained wound-healing response, leading to excessive deposition of extracellular matrix (ECM) proteins. As discussed previously, fibrosis represents the accumulation of scar tissue within the liver. When this process persists, fibrotic bands expand and reorganize into widespread scarring, accompanied by the formation of regenerative nodules. This marks the progression to cirrhosis, where normal liver architecture becomes severely distorted. These structural changes increase resistance to blood flow and impair hepatocyte* function, contributing to complications such as portal hypertension and hepatic insufficiency.
Hepatorenal syndrome is a serious complication of advanced liver disease where the kidneys stop working properly, even though they are not physically damaged. In Budd–Chiari syndrome, poor blood flow through the liver causes changes in circulation throughout the body. Specifically, blood vessels in the abdomen widen which reduces the amount of effective blood reaching important organs.
To compensate, the body activates systems that tighten blood vessels, especially in the kidneys. This reduces blood flow to the kidneys, making it harder for them to filter waste from the blood. Over time, this leads to kidney failure. This condition shows that the liver disease has become severe and is associated with a poor outcome.
Hepatic encephalopathy is a neuropsychiatric complication of liver dysfunction characterized by altered mental status, confusion, and, in severe cases, coma. In Budd–Chiari syndrome, impaired hepatic blood flow and liver dysfunction reduce the liver’s ability to detoxify substances in the blood. As a result, toxins such as ammonia accumulate in the systemic circulation. This buildup of ammonia disrupts normal brain function and contributes to neurological impairment. Hepatic encephalopathy reflects advanced liver dysfunction and is associated with poor clinical outcomes.
The liver undergoes characteristic histological changes as Budd-Chiari syndrome progresses from acute congestion to chronic cirrhosis. These structural changes drive the development of major clinical complications over time.
Early (Days to Weeks)
Hepatic venous obstruction causes congestion and increased pressure, leading to portal hypertension 15.
Potentially reversible
Intermediate (Weeks to Months)
Ongoing injury and reduced oxygen promote liver fibrosis, causing scarring and worsening blood flow 9,14.
Partially reversible (only early stages of fibrosis)
Advanced (Months to Years)
In the final stages, cirrhosis develops as a major complication, leading to severe liver dysfunction 16.
